While Inscryption is a deck-building game, the player can find and collect multiple items to help them on their journey. These range from providing cards to giving the player an edge over Leshy. Some items are better than others and you can only hold three at a time normally.
The Squirrel in a Bottle is a great card as it provides you with a free sacrifice. The Hourglass, Fan, and Piggy Bank are also great choices. The Dagger, Fishhook, and Scissors are good items but must be used in the right situation. You can see all items and their effects below.

| Bottle | The player can receive bottles that have various cards in them. Using this item places the card in the players hand. These cards can be Squirrel, Boulder, Blood Goat, and Frozen Opossum. | From Backpacks |
| Pliers | Pulls one of your teeth out and places on Leshy's side of the scale, lowering it by one point. | From Backpacks |
| Fan | Gives all cards on your side the Flying sigil for one turn. | From Backpacks |
| Scissors | Cuts up one of Leshy's cards, removing it from the board. | From Backpacks |
| Piggy Bank | Gives the player four bones. | From Backpacks |
| Hourglass | Forces Leshy to skip their turn. | From Backpacks |
| Fishhook | Allows to the player to steal one of Leshy's cards and place it on their side. This only works if the player has an open space and can only be used once per run. | Defeating the Angler |
| Dagger | Carves out your eye and places it on Leshy's side of the scale, lowering it by four points. Can only be used once per run. | Solving the Caged Wolf Puzzle |
| Green Blob Bottle | Leshy will remove this item from the board if you try to use it. He places it on the shelf behind him. It will give hints but otherwise has no purpose. | From Backpacks |
